Our Service

The Young Stroke Service provides assessment, referral and treatment options for people recovering from stroke.

We can also provide specalised guidance about young stroke to clinicians.

We do not replace other services in the community.

Confirming referral

The Young Stroke Service team will contact the referrer to confirm the referral.

Call with the Young Stroke Service

Our first call with the young stroke survivor will tell them what our services do, answer any questions they have and set up their intake appointment.

Intake appointment

An intake appointment happens with our clinical therapist. It involves collecting personal and medical information about their stroke, current symptoms and previous treatments.

The Young Stroke Service multidisciplinary team will consult following the appointment on what we can offer.

The referrer will get a copy of the recommendations.

Refer to the Young Stroke Service

Young stroke survivors require a referral from a health professional to access the Young Stroke Service.

This can include:

  • a GP (doctor)
  • a health professional in the hospital (neurologist or nurse)
  • a health professional in the community (physiotherapist or psychologist)
